An ADHD evaluation looks at multiple factors in your life and cognitive functioning to determine if a diagnosis of ADHD is warranted. The ADHD assessment includes a comprehensive clinical interview, self-report surveys, rating forms/surveys for parents and teachers, collateral communication with your child/teen's teachers, a computerized test, and a cognitive functioning assessment. Autism Spectrum Disorder (ASD) Evaluation is a comprehensive assessment that looks at multiple factors, including developmental history, to see if an ASD diagnosis is appropriate. Dependent on age, this evaluation will include a combination of the following: comprehensive clinical interview, self-report rating forms/surveys, rating forms/surveys for parents and teachers, collateral communication with your child/teen's teachers, and a cognitive functioning assessment. Psychodiagnostic Assessments help determine an appropriate diagnosis by assessing your psychological functioning and personality traits. This type of assessment includes a comprehensive clinical interview and self-report surveys. Additional rating forms/surveys may be collected from family members. For children and teens, rating forms/surveys for parents and teachers and collateral communication with your child/teen's teachers Medical/Surgical Clearance Assessments help determine your strengths and potential challenges prior to a major surgery (such as an organ transplant, bariatric surgery, or gender-affirming surgery). This evaluation also provides recommendations for psychological care pre/post surgery. In most circumstances, your medical team or current healthcare system will conduct the evaluations. However, in the rare circumstance that you are in need of an assessment, I am able to conduct these evaluations. Psychoeducational Assessments are used to identify academic, cognitive, and social emotional strengths and challenges in the academic setting. These assessments also include assessing for specific learning disabilities. This type of assessment includes a comprehensive clinical interview, self-report surveys, collateral communication with teachers for children/teens, an achievement assessment, a cognitive functioning assessment, and rating forms for teachers.
